COVID-19 devastated our care infrastructure, and millions left the workforce to provide care in home. As baby boomers age into retirement and increase demand for home care services, quality care becomes harder for families to find and afford. Now more than ever, families are compelled to provide increasingly complex care in the home as a safer, more affordable option for long-term care.
At Aidaly, we're on a mission to reimagine in-home care for our most vulnerable populations. Our unique model puts family caregivers at the center of care delivery and addresses SDoH from within the household. Aidaly provides family caregivers with the training, support, and compensation they need to deliver high-quality care in the home. With a combination of technology & human support, Aidaly’s platform empowers families to keep their loved ones living in their homes and communities longer while filling a critical homecare workforce need.

Responsibilities

  • Work directly with leadership to shape and execute our customer success strategy from the ground up
  • Conduct health-based assessments and create care plans
  • Complete mental health care for up to 25-30 patients per week
  • Document and operationalize all interactions with patient
  • Provided and maintain quality clinical oversight and ongoing coaching to family caregivers
  • Create and conduct group care sessions
  • Maintain clinical systems, clinical meetings, and procedures
  • Build relationships by attending community events and resource fairs
  • Help engage in marketing to grow our patient panel
  • Build care education; patient quality improvements initiatives
  • Be the voice of the patients and share feedback with our executive team and cross functional leads (sales, product, marketing, etc.)
  • Work cross-functionally to support company-wide initiatives
  • Leverage technology to scale the impact of your time

Qualifications

  • Active Licensed Clinical Social Worker in Florida,
  • Master's level degree and licensure (LCSW) and National Provider Identifier (NPI); in good standing with licensure
  • Experience working in a healthcare setting and telehealth systems
  • Knowledgeable about treatment planning, plan of care, and follow up
  • Experience with mental health and/or substance abuse diagnosis
  • Comfortable with geropsychology, family therapy, and cognitive behavioral therapy
  • Experience working in partnership with clients to achieve goals
  • Ability to utilize comprehensive assessments
  • Strong organizational skills to develop processes and systems including excellent executional skills and attention to detail
  • Extreme ownership and pride in your work
  • Mission-driven and empathetic
  • Comfortable with technology, including telehealth software, video communication, and internal communication tools
  • Meets background/regulatory requirements
  • Able to work in the United States
  • Willing to work in different states
  • Proficiency in English and Spanish
  • Some remote work: Work from home space must have privacy for patient safety and HIPAA requirements
We will interview in two stages: Interview one will focus on social advocacy. Interview two will focus on market growth and strategic planning.
